First, review the list below and select up to three intercepts from the Sequential Intercept Model (SIM) that you'd like to focus on during the current assessment.
Next, this application will ask you to answer up to 20 questions about your community's current approach to mental health issues at those intercepts. After answering the questions, you'll receive a report highlighting which building blocks of infrastructure you should focus your immediate efforts on and offering a list of resources that can help you in your efforts.
